Part A Terminology

Part C Individualised Plans

11. You are to provide details of 3 individualised plans used as the basis to support three different clients , with whom you have or are working.

You are to refer to the clients as Client 1, Client 2 and Client 3. Do not use their correct names.

You are to provide details about the following features of their plans and your actions/activities to support their plan.

  1. Client details: age, gender
  2. Location of plan: eg residential care facility, group home, juvenile justice facility, client’s own home
  3. Short case history: approximately 50 words outlining the client’s history, cultural background, current status – cognitive abilities, physical health , the client’s priorities
  4. Purpose of plan: Why has the plan been developed? What are the agreed outcomes?
  5. Who was involved in the planning process? (no names - their relationship to the client only, such as son, speech therapist, physiotherapist, respite carer, counsellor)
  6. How much was the client consulted during the planning process?
  7. What strategies have been suggested to meet the agreed outcomes?
  8. What resources have been allocated to support the strategies?
  9. Are there any infection prevention or control procedures you need to follow?
  10. What activities/actions you have been involved in to support the client’s plan? (frequency, duration, review periods)
  11. What timeframes were suggested for these activities/actions? Where the timeframes adequate to support the client needs?
  12. Are other service providers involved in the client’s plan?
  13. What do you perceive as the client’s s trengths and interests? Are these adequately addressed in the individualised plan?
  14. Can you identify any risks to the client, yourself or others due to the activities/actions required to support the plan?
  15. Instances when have you discussed with the client and your supervisor, strategies to support the client’s self -determination, and skill maintenance and development?
  16. Are there any unmet needs or have new needs been identified?
  17. How is the individualised plan reviewed?
  18. What organisation reports and documentation are you required to complete and maintain? How are these documents stored?

12. Which is more important to take into account, the preferences of the family or the person you are supporting? Explain your reasons.

13. Explain what you would do in relation to your duty of care if you think a client may be harmed from taking part in activities.

14. Give an example of a strategy that you would use to help minimise an environmental hazard such as slippery or uneven floor surfaces.

15. Describe three (3) ways you can ensure you maintain a comfortable environment for a person receiving support.

16. Explain why Human Rights are central in respecting individual support when providing support services. (minimum 50 words)

17. Give three (3) examples of changes in life circumstances that may require reporting and support.

18. Why is it necessary to seek assistance when it is not possible to provide appropriate support? Illustrate your answer by providing two (2) examples.

19. How would you monitor your own work to ensure that the required standard of support is maintained?

20. In what way would you as a support worker for an aged and/or disabled person participate in discussion with the client and supervisor to identify areas of the individualised plan that requires review?

29. Why is it important to document in a manner that is consistent with reporting requirements?

30. You have been entrusted with a new employee as part of your job today. You need to explain to the new employee about the file documentation in regards to organisational policies and procedures. Provide five (5) dot points ion what you would tell the new employee in regards to file documentation.

31. Complete the table below. Read the situation and then state (Yes or No) whether the worker respected the person’s culture and explain your answer.

Situation

  1. The male support worker on duty organised for a female support worker to assist a Muslim female client with her personal care needs.
  2. The support worker organised the services of an interpreter after the client wanted to speak in their own language when reviewing their care plan.
  3. The support worker made assumptions about the client’s cultural practices and needs on the basis of their language.
  4. The support worker organised a family case conference without the services of an interpreter, even though the 82 year old Polish client spoke very little English.

32. Case Study 1

Read the following case study and answer the questions.

Mr Turner is vision impaired and has mobility issues due to a previous fall. Your job role is to provide Mr Turner with personal care. You notice that his bathroom has not been cleaned in a while and there is dirty laundry and towels on the floor. You also notice that in the kitchen there are many dirty dishes and food remnants on the bench which are attracting ants. After the shower you assist Mr Turner with dressing and notice that his clothes smell of urine and look like they have not been washed in a while. You ask Mr Turner whether he needs assistance with tidying and cleaning, however, he states that he is fine and he manages OK and that his family assist him when they are available. According to Mr Turner’s individualised plan you are only to provide assistance with personal care.

  1. What should you do in this situation whilst maintaining Mr Turner’s rights, and your duty of care?
  2. How would you address the issues you have seen with Mr Turner?
  3. How would you report this situation?
  4. When should Mr Turner’s family be informed of his present situation?
  5. When providing personal care for Mr Turner, what equipment may you need to assist with the task?

33. Case Study 2

Read the following case study and answer the questions.

Care Plan

Client Name: Mrs. Kaur

Goals:

  1. Regular house cleaning, laundry and cooking.
  2. Development of a meal plan to maintain adequate nutrition.
  3. Assistance and transport for weekly supermarket shopping.
  4. Community and home-support worker (funded).
  5. Regular foot-care ie toe nails cut, corns and other lesions monitored, treated and dressed Read the goals in the above care plan and briefly describe the activities, skills and health specialist or service provider (s) that will be required to assist in Mrs. Kaur achieving the goals outlined in her care plan.

For example: House cleaning assistance, laundry and cooking may be provided by a support worker, or a professional cleaning service.

Read the following case study and answer the questions.

Before his disability, Johnson was active in sports including basketball. Johnson who is now in a wheelchair would like to play basketball again. A fellow support worker says no to his request as he thinks Johnson will not enjoy it and believes that he might injure himself.

  1. What are you going to do in this case besides reporting this to the supervisor?
  2. Is there anything you need to do in relation to the Johnson’s individualised plan?
  3. How can you ensure that Johnson is given the opportunity to enjoy basketball?
  4. If Johnson wanted to complain about the other support workers response to his suggestion of playing basketball, what would you suggest?
