HCMG850 Health Care Reform : Decision Making Model

1.The first step of the decision making model is to access whether there is an issue related to quality of the service. In the given case, the district hospital has faced issues due to poor service quality in the emergency department.  The organization also lacks sufficient information to make quality decisions as the doctors in the emergency department do not have effective leadership power to deal with the issued that are faced by the nurses and other care workers within the department. The lack of proper information is considered to be the main cause of this issue. The problem faced by the district hospital can be structured using the primary information that is available.

In order for the effective function of the emergency department, it is curial for all the sub-ordinate members to have active participation in the decision making process of the organization. In this case, the doctors need to respect the decision taken by the nurse and other workers, who are employed in the emergency department. These subordinates members need to have active participation in the matter of solving issues that are faced by the emergency department. It is also the duty of the hospital management to ensure that there is no conflict within the subordinate employees.

2.The total number of patients in the emergency department of the district hospital has gone down significantly. According to the data of the impatient census, the total occupancy of the district hospital in the emergency ward is just 42%. It is essential for the hospital management to provide effective training, which will help the hospital staffs to maintain communication with the physicians. In all hospitals, the emergency department has to deal with critical cases and thus, it is important to have effective level of commutations among the workers. It is also important to provide high level of financial support in the emergency department, which helps to deal with critical care that is needed for the same.

3.The principles of CQI are based on the fact that it is better to have border term of investment rather than reducing the short-term costs of the emergency department (Krol et al., 2014). It is also essential to have a protocol that is based on improving the care quality and customer satisfaction. The Deming Chain Reaction that is present within the principles of CQI emphasizes on the fact that it is important to have more important to focus on improved quality of service that is usually obtained by decreasing the cost of the service (Ramshaw et al., 2016). As the cost price is decreased, the productivity can now easily be increased along with the share of the market. Hence, the emergency department of the district hospital can have better quality of service by providing more jobs to the people. As the total number of employees within the emergency department increases, there is more chance of providing satisfactory quality of service. Hence, the district hospital can expect to get better return on investment.

4.The EMTs are considered to an integrated part of the emergency department of the hospitals. The EMT programs help in providing the ambulance service to the district hospitals. The EMTs also help in the quick transportation of the critical patients to the hospitals and thus is able to provide better quality health service (Drachsler et al., 2016). With the help of the EMTs, the district hospital is thus able to provide good quality emergency care to the patients.

It is the duty of the physicians in the emergency department to deal with the activities of EMTs, which will help them in the better quality of health and emergency care service. It is also the duty of the physicians to provide proper education for this purpose. However, the main drawback of this educational program is that it does not provide any practical skills, which is an important part of the emergency medical care service.

5.Few of the major steps that are taken to the District hospital are to improve the quality of education that is provided to the staffs. The development of EMT program is one of the major types of initiative used for this purpose. The management of the hospital has organized interactive sessions to the physicians and other working staffs, which helps in the matter of improved coordination among within the department.  The interdepartmental education program is also one of the important steps that is used in this context.
