Factors that can lead to create risks of harm, abuse and failure to protect for individuals
The risk of someone harming himself or herself can come from different factors. This is the same for the case of the abuse, neglect and failure to protect of service users. The case of Robert who is someone mental health problem make is easy to see how the he can still be depending to other people regardless of his advance age. There is a need to help them to do the day-to-day activities. Due to many reasons, the people with mental disabilities have suffered discrimination and have the risk of being neglected. This situation can also lead to the risk of them being abused or harmed in one point in their lives. By suffering this way in the community, the may have problem in getting involved in many social activities which can lead to their social isolation according to Adams (2008). Due to their isolation and other factors, people with mental disabilities can show aggressiveness that many of us can be looked at as anti behaviour. People with mental disability need to be care for accordingly to avoid the risk of problem such as physical abuse. For the care workers, their cares can also lead to injuries and abused also neglects sometime old people. The staffs need to identify the risk of harm an abused of old people. This can be considered a failure for them being protected.
The management needs to set up a mechanism to be able to prevent the risk of any abused. The UK Safeguarding Act for adult gives us the necessary grounding to prevent the harming, the abuse and the negligence of patients in the health and social care. There is a need for the service users to be kept private so they could keep their dignity. That is why the management needs to promote a good communication between the carers and the patients to minimise the risk of abuse or negligence. This same type of communication should also be promoted in an inter-professional level within the health and social care sector. By doing so, it would be easier to achieve high standard of service provision. Cares or anyone else in the healthcare environment should be able to identify potential health hazard (Concannon, 2009). . At the end, the management should be able to identify the factors of risk that can create problem such as abuse or negligence.

In recent times, the NHS of United Kingdom has been constantly facing problems within their department. The health sector has constantly been facing criticism from several aspects and been in a constant degradation. Both the group staffs and patients are at risk by various ways in the organization (telegraph.co.uk/news 2017).
Individual risk and failure to protect:
The chosen organization has been coming up in the news repeatedly in recent times. They have been unable to cater to the people’s demand. According to the news published by the Star in February 2017, the hospital has rejected around 1500 operations in the last year. It has been seen that itself in December 82 operations has been cancelled by the hospital authority (thestar.co.uk/news 2017). Most of the time, the authority blamed it on the unavailability of proper facility within the organization specially beds and theatres. As a result, patients are deprived of their rights to good health.
The individuals are also facing problems other aspects like incorrect medications. It has been seen that few individuals have been given wrong medicines and as a result, they have been victim of adverse impact of this medicine (dailymail.co.uk 2017).
There has been report that the nurses in some of the Sheffield’s organization have been the victim of abuse within their organization. Earlier history suggests that in one of the organization under NHS one nurse have been severely injured as a result of the abuse faced in the organization. This implies that the organizations lacks in regulation that protects its employees.
The unique problem faced by Sheffield is that of delay in their mode of operation. It is the right of each individual to get timely health care service. It has been observed that people has constantly kept on lodging complaint against the NHS. Complaints has been registered against Sheffield stating that it kept some of its patients waiting for several months before they can get an appointment with their respective doctors for treatment (thestar.co.uk 2016). Here the patients faces risk of their life. In regards to this issues the organization justified themselves by stating that none of the patients who had been delayed has any serious issues like cancer. This clearly shows the carelessness and casual attitude of the administrative system of the organization.
Other than, the highlighted issues mentioned above, there has also been the case where proper investigation lacked for unnatural deaths of the patients of a large number of patients (telegraph.co.uk 2013). Even the consumers have been deprived of their rights to get proper information regarding those cases. Shortage of efficient staffs in the hospital also deprived the patients of UK in getting proper treatment (kingsfund.org.uk 2016). In addition, the lack of joint ventures to integrate the different segments of the healthcare unit also poses problems in operation thereby depriving consumer of their rights. The patients also faces discrimination, risks of getting higher bills charged and wrong medications.
Effectiveness of Policies for promoting managing risks:
In order to manage the risks that the patients face in several forms the government of UK established few governing bodies that can control these issues and manage the risks faced by an individual and the employees of the health sector. In order to manage risks the first thing that needs to be done is to raise awareness within the organization and to spread the news to the patients and their family (sth.nhs.uk 2012). The people should be provided with detailed information regarding the steps that can be taken if they feel they are deprived of their rights. Even prior to awareness raising the exact department, which poses the maximum threats, needs to be identified. It is a rigorous process that needs to be followed daily within the organization. Sheffield under NHS trust have taken up different policies and strategies to mitigate the risks in different departments namely, childcare, maternity and training departments (bbc.com 2017). Though the government of UK through NHS has taken up several risk management measures but still now the country faces serious problems in this sector. According to a reputed daily, it has been observed that almost half of UK’s population thinks that the issues of NHS are a cause of concern. Michael Clemence states that the strikes and incapability to handle pressure still exists and have reached a new height. According to Chris Ham, it has been high time when the ministers need to intervene and control this situation to restore faith in people’s mind (onmedica.com 2017). Another article suggests that in spite of reducing or mitigating the risk, the hospitals are increasing it by implementing apps and highly efficient technologies in place of human beings as staffs. The lack of fund is also another reason, which restricts the successful implementation of policies within the health industry (healthknowledge.org.uk 2017).
Conclusion:
The entire report highlighted only one single fact that there is existence of chaos and anarchy within the health care industry of United Kingdom. People have been constantly depriving of their rights to good health by various means and hence the government has taken up measures to reduce this problem within the economy. From task 1 it can be concluded that people have the right to get service irrespective of their ethnicity, sex, religion and other discriminating factors. The users are entitled for getting privacy within the organization whereby their cases should not be disclosed and discussed amongst others. It has also been observed that communication plays the crucial role in managing and running the entire system of health care. From the case study in the second task, it has been seen that Mr. Luther Blisset faced issues with Sheffield hospital regarding the administration, staff behavior, availability and conditions of the bed and other services. This shows that there has been existence of negligence in part of the authority. Through task 3 we came to know that the basic rights of human beings have been snatched away from them by the existing system in UK. In order to get proper health care facilities people often needs to wait for long period of time, they faces harassment in the form of wrong treatment, deprivation due to unskilled staff and are prone to danger due to use of unhygienic tools. At last, task 4 provides a memo that has been targeted to bring in the lost discipline within the system through official announcement of the policies and making people aware.
